Digestive enzymes are the proteins that turn a meal into molecules your body can absorb. This guide explains what they are, which enzyme handles starch, protein, fat and lactose, what the clinical trials actually show for supplements, and how to read a label before you buy one in Ireland.
Digestive enzymes are proteins made by the salivary glands, stomach, pancreas and small intestine that break starch, protein and fat into absorbable units. In Ireland they are sold as food supplements regulated by the Food Safety Authority of Ireland (FSAI) under EC Regulation 1924/2006, not as medicines, with 13.5% VAT. The only digestive enzyme with an authorised EU health claim is lactase, at a minimum of 4,500 FCC units per lactose-containing meal (Regulation 432/2012). The strongest clinical evidence is for prescription pancreatic enzyme replacement in diagnosed exocrine pancreatic insufficiency, where a 2017 Gut meta-analysis of 17 trials (de la Iglesia-Garcรญa et al., PMID 27941156) found fat absorption of 83.2 versus 67.4 on placebo (p=0.0001). In healthy adults the evidence is limited: one small crossover trial found a modest bloating reduction after a high-fat meal (Suarez et al., 1999, PMID 10489912), and alpha-galactosidase reduced bean-related gas in healthy volunteers but not in adults with irritable bowel syndrome. Within prescription pancreatic enzyme preparations, no product or delivery system was shown superior (Taylor et al., 2010, PMID 19804466), and no trial has compared over-the-counter enzyme brands against each other. Over-the-counter digestive enzyme products, including the pancreatin in NOW Super Enzymes, are food supplements. They are not equivalent to prescription pancreatic enzyme replacement therapy, which is a licensed, enteric-coated, standardised medicine used under medical supervision for diagnosed exocrine pancreatic insufficiency. The strong clinical evidence discussed on this page belongs to that prescription category. What is well-supported: Prescription pancreatic enzyme replacement improves fat absorption in diagnosed exocrine pancreatic insufficiency (de la Iglesia-Garcรญa et al., Gut, 2017, PMID 27941156; 17 RCTs, 511 patients).
What is not proven: That over-the-counter enzyme blends improve digestion or nutrient absorption in healthy adults. The healthy-volunteer and functional dyspepsia trials are small (n=18 to n=40) and one is manufacturer-funded.
Most relevant human dose range: One capsule or tablet with each meal, as labelled. Alpha-galactosidase trials used 300 to 1,200 GalU per meal; the EU lactase claim requires at least 4,500 FCC units per meal.
Key safety note: Bromelain and papain are unsuitable for pineapple or papaya allergy and the NOW label advises a doctor's advice if taking blood thinners. Betaine HCl acidifies the stomach and is not appropriate for anyone with ulcers, gastritis or reflux without medical advice.

What digestive enzymes are and where the body makes them
Digestive enzymes are proteins that speed up specific chemical reactions involved in breaking down food. The reaction they catalyse is hydrolysis, which uses water to split a large food molecule into smaller pieces. Without them, a steak or a bowl of porridge would pass through you largely unchanged.[1]
Digestion starts in the mouth. Salivary amylase begins splitting starch while you chew. In the stomach, pepsin starts on protein, and it only works in the acid environment that stomach cells create.
Most of the work happens in the small intestine. The pancreas releases pancreatic amylase, the proteases trypsin and chymotrypsin, and pancreatic lipase into the duodenum. The liver adds bile, which is not an enzyme but emulsifies fat so lipase can reach it.
The final step happens on the surface of the intestinal lining, where brush-border enzymes such as lactase, sucrase and maltase split disaccharides into single sugars. A person with low lactase activity cannot finish that job for milk sugar, which is why lactose intolerance exists.
The main digestive enzymes and what each one digests
The simplest way to understand enzyme supplements is to learn what each enzyme acts on. The name usually gives it away: the suffix "-ase" is attached to the substrate. Lactase acts on lactose, lipase on lipids, protease on protein.

Two things follow from this table. First, humans do not produce alpha-galactosidase or cellulase, so raffinose-family sugars and cellulose reach the colon undigested, where bacterial fermentation can produce gas. That is why beans and fibrous vegetables are common gas triggers. Second, most "broad-spectrum" supplements are built around pancreatin, a porcine extract that supplies amylase, protease and lipase in one ingredient.[1]

A healthy pancreas produces enzymes in large excess. Fat malabsorption only becomes clinically obvious when lipase output falls to a small fraction of normal, which is why most people digest a rich meal without any help at all.[1]
Enzyme supply does fall short in specific situations. Exocrine pancreatic insufficiency, seen in chronic pancreatitis, cystic fibrosis and after some pancreatic surgery, is the clearest example. It is a medical diagnosis, treated with prescription pancreatic enzyme replacement therapy, and it is where almost all the strong evidence sits.[2]
Lactase is the other well-defined shortfall. Most adults worldwide lose much of their lactase activity after childhood, and the resulting lactose maldigestion is the target of the one EU-authorised enzyme health claim.[13]
For otherwise healthy adults, occasional fullness after a large meal or gas after a bean chilli is not, by itself, evidence of an enzyme deficiency. Such symptoms more often reflect meal composition and normal colonic fermentation, although they can have other causes. That everyday situation is what over-the-counter blends are sold for, and it is where the evidence is thinnest. Persistent, severe or changing symptoms should be assessed by a GP.
Pale, greasy or floating stools, unexplained weight loss, persistent diarrhoea, or symptoms that started after pancreatic disease or surgery can indicate true enzyme insufficiency. These are assessed with tests such as faecal elastase and treated with prescription enzymes at doses far above anything in a supplement. Do not self-treat these with an over-the-counter product.
What the clinical evidence shows
This is where most supplement pages overreach. The honest picture is a strong body of evidence in one clinical setting and a small, mixed body of evidence in everyone else. Both halves are set out below, including the trials that found nothing.
Prescription pancreatic enzymes work in diagnosed pancreatic insufficiency. de la Iglesia-Garcรญa et al. (NIHR Liverpool Pancreas Biomedical Research Unit, Gut, 2017) pooled 17 randomised trials in 511 patients with chronic pancreatitis. Pancreatic enzyme replacement raised the coefficient of fat absorption to 83.2 versus 67.4 on placebo (p=0.0001) and improved nitrogen absorption, stool weight and abdominal symptoms.[2]

No prescription formulation was shown superior, and over-the-counter brands have never been compared. Taylor et al. (Aliment Pharmacol Ther, 2010) reviewed the pancreatic enzyme trials in exocrine pancreatic insufficiency and concluded that enzyme preparations improve fat malabsorption versus placebo, but that no specific product or delivery system consistently outperformed another.[3] That review does not compare or validate over-the-counter enzyme blends; no head-to-head trial of supplement brands exists. Both points are worth keeping in mind whenever a label claims a proprietary blend is "the strongest".

A modest effect in healthy people after a high-fat meal. Suarez, Levitt et al. (Dig Dis Sci, 1999) gave 18 healthy volunteers pancreatic enzyme capsules or placebo with a 1,196 kcal, 72 g fat meal in a double-blind crossover. Enzymes reduced bloating over the recording period (p=0.049), with reductions in gas and fullness in the evening, but had no effect on breath hydrogen or methane.[4] The p-value sits right at the threshold and the trial has never been replicated at scale.

Symptom improvement in functional dyspepsia, with caveats. Majeed et al. (J Med Food, 2018) randomised 40 patients to a fungal multi-enzyme complex or placebo three times daily for 60 days and reported significantly better dyspepsia scores in the enzyme group (p values 0.0401 to 0.0033) with no product-related adverse events.[5] The trial was funded and run by the ingredient's manufacturer. A second, independent Italian trial (Ullah et al., Biomed Pharmacother, 2023) used a fungal blend of protease, amylase, lipase, cellulase and lactase and also reported lower symptom and pain scores than placebo.[6] Both are single-centre and small.

Alpha-galactosidase reduces bean-related gas in healthy volunteers and children. Di Stefano et al. (University of Pavia, Dig Dis Sci, 2007) gave 8 healthy volunteers 300 or 1,200 GalU of alpha-galactosidase or placebo with 420 g of cooked beans; the 1,200 GalU dose significantly reduced breath hydrogen and flatulence severity.[7] Di Nardo et al. (Sapienza University of Rome, BMC Gastroenterol, 2013) randomised 52 children with gas-related symptoms for two weeks and found fewer days of moderate-to-severe bloating (p=0.03) and less flatulence (p=0.02) versus placebo.[8]

The same enzyme did nothing in adults with irritable bowel syndrome. Bรถhn et al. (University of Gothenburg, Neurogastroenterol Motil, 2021) ran a double-blind crossover pilot giving 1,200 GalU of alpha-galactosidase or placebo with three oligosaccharide-rich meals to IBS patients. The enzyme was not superior to placebo for gas, bloating or any other symptom, and the authors concluded it cannot be recommended to adult IBS patients for meal-related symptoms.[9]

The evidence gap for healthy people is real. Outside diagnosed pancreatic insufficiency, the trials are small (8 to 52 participants), short (one meal to 60 days), use different enzyme blends and doses, and in one case were run by the manufacturer. There is no large, independent trial showing that a broad-spectrum enzyme supplement improves digestion, nutrient absorption or comfort in healthy adults eating a normal diet. Everyday use is low-risk and reasonable for people who notice a difference, but it is not evidence-based in the way prescription enzyme therapy is.
What that means in practice
If a product helps you after a heavy meal, the mechanism is plausible and the safety record is good. If it does nothing, that is consistent with the evidence too. The one clear signal is that targeting the right enzyme to the right food matters more than total enzyme count: alpha-galactosidase for beans, lactase for dairy, broad-spectrum blends for mixed meals. Enzymes act on the food in front of them. Trials measured effects within the same meal period: Suarez 1999 recorded symptoms for 15 to 17 hours after a single dose; Di Stefano 2007 measured breath hydrogen for 8 hours. Any effect is felt on that meal, not cumulatively.
The Di Nardo 2013 paediatric alpha-galactosidase trial ran for 14 days and measured bloating days and flatulence. Two weeks is a reasonable trial period to decide whether a targeted enzyme suits you.
The Majeed 2018 functional dyspepsia trial dosed three times daily for 60 days with no product-related adverse events. This is the longest published supplement-blend trial; there is no long-term safety data beyond it.

Betaine HCl and ox bile: what a "super" enzyme blend adds
Products such as NOW Super Enzymes add two non-enzyme ingredients to the pancreatin, bromelain and papain base. Both are worth understanding before you buy, because they change who the product suits.
Betaine HCl
Betaine hydrochloride is a salt that releases hydrochloric acid when it dissolves in the stomach. Yago et al. (University of California, San Francisco, Mol Pharm, 2013) gave 1,500 mg to six healthy volunteers whose stomach acid had been suppressed with a proton-pump inhibitor. Gastric pH fell from 5.2 to 0.6 within about six minutes and stayed below 3 for roughly 73 minutes.[10]
That is a pharmacology result, not a clinical one. It shows betaine HCl acidifies the stomach; it does not show that doing so improves digestion or symptoms. A 2020 review by Guilliams and Drake concluded that evidence for meal-time betaine HCl in people with suspected low stomach acid is limited and that the popular "self-test" protocols are unvalidated.[11]
The practical consequence: a betaine HCl blend is the wrong choice for anyone with reflux, gastritis, an ulcer history, or who takes acid-suppressing or anti-inflammatory medication. Adding acid to an irritated stomach is not a good idea, and this is the most common reason people report a burning sensation from "super" enzyme products.
Ox bile
Ox bile extract supplies bile salts, the detergents the liver normally makes to emulsify fat. In a blend it is intended to complement lipase on fatty meals. The trade-off is that bile salts can loosen stools, particularly when the meal is low in fat, and the ingredient is bovine, which rules the product out for vegetarians.

Digestive enzymes vs probiotics vs prebiotics
These three are shelved together and constantly confused, and "digestive enzymes and probiotics" is one of the most common searches in this category in Ireland. They are not interchangeable.

Enzymes act during the meal and are gone by the next one. Probiotics are living cultures with their own storage requirements, often refrigeration, and their effects are studied over weeks. Prebiotic fibres such as inulin and PHGG are, by definition, the things enzymes cannot break down.
The two can be taken together. Combined "enzymes plus probiotics" capsules exist, but they typically carry a low dose of each. How to read a digestive enzyme label
Enzyme labels are harder to compare than vitamin labels because the useful number is activity, not weight. Five things decide whether a label is telling you anything.
Activity units, not milligrams. Enzymes are declared in units such as USP, DU, HUT, GDU, FCC PU, LU and GalU. A milligram figure says how much powder is in the capsule, not how active it is. Two products can only be compared on the same enzyme in the same unit.
Units do not convert. A GDU (bromelain) cannot be turned into an FCC unit or a USP unit. If a label mixes units across enzymes, that is normal; if it gives one blended weight with no units, it is telling you very little.
Source decides suitability. Pancreatin and pepsin are porcine, ox bile is bovine. Pancreatin in a food supplement is not prescription pancreatic enzyme replacement therapy; prescription products are enteric-coated, standardised medicines. Bromelain, papain and fungal enzymes (from Aspergillus species) are plant or microbial and suit vegetarians and vegans. Check the capsule shell as well: some are gelatine, some are plant-based hypromellose.
"Acid-stable" matters for fungal proteases. Some fungal proteases are labelled acid-stable, meaning they can start working in the stomach rather than waiting for the small intestine. Pancreatin, by contrast, is inactivated by acid and works in the duodenum.
Higher is not better. In prescription pancreatic enzyme preparations, the one setting where enzymes are well studied, Taylor et al. found no formulation consistently superior to another.[3] Over-the-counter blends have never been compared head-to-head. A bigger activity figure is a formulation difference, not evidence of a better product.
How to take digestive enzymes: timing, duration, side effects
When to take them
With the meal, at the start of eating. Enzymes work on the food they are mixed with, so a capsule taken an hour before or after a meal has nothing to act on. Most labels, including NOW Super Enzymes, direct one capsule with a meal. Targeted enzymes such as alpha-galactosidase or lactase are taken with the specific trigger food and are pointless with other meals.
How often and for how long
Follow the label and do not exceed the stated daily dose. Use them with the meals that cause trouble rather than every meal by default. Published trials of supplement blends ran from a single meal to 60 days without product-related adverse events.[5] There is no long-term safety trial beyond that, so the honest answer to "how long can I take them" is: as directed, and if you still need them daily after a couple of months, see a GP.
Side effects and who should be careful
Digestive enzymes are generally well tolerated. The reported issues are mild and mostly predictable from the ingredients.
- You have a diagnosed pancreatic condition. Exocrine pancreatic insufficiency is treated with prescription enzymes, not a supplement.
- You have coeliac disease. Gluten-targeting enzymes do not make gluten safe; a strict gluten-free diet under HSE and dietitian guidance is the only safe approach.
- You have reflux, gastritis or an ulcer history, or take acid-suppressing or anti-inflammatory medication. Betaine HCl blends add acid.
- You take blood-thinning medication. The NOW Super Enzymes label advises consulting a physician; bromelain and papain are the ingredients of concern.
- You are allergic to pineapple or papaya, the sources of bromelain and papain.
- You react to sulphites. NOW Super Enzymes contains sulphites.
- You are pregnant or breastfeeding, or under 18. NOW Super Enzymes is labelled for adults aged 18 and over.
- Whether you have exocrine pancreatic insufficiency, coeliac disease, gallbladder disease, reflux disease or a food intolerance. Those are diagnoses, made by a doctor with tests.
- Whether your symptoms are caused by low enzyme output. Meal-related discomfort has many causes and an enzyme supplement is not a diagnostic test.
- Whether a supplement is safe alongside your medication. Ask a pharmacist or GP.
Nausea or loose stools are the most commonly reported effects, usually from ox bile with a low-fat meal or betaine HCl on an empty stomach. Stop the product if you react to it. Gluten-targeting enzymes deserve one more caution: a 2018 single-blind crossover trial (Ido et al., Clin Transl Gastroenterol) found a combination of gluten-digesting enzymes improved symptoms in people with non-coeliac gluten sensitivity, but that population had coeliac disease excluded first, and no enzyme product protects anyone with coeliac disease.[12]
Digestive enzymes in Ireland: regulation, the lactase claim, VAT and buying
Digestive enzyme supplements are legal in Ireland and sold as food supplements under EU food law, overseen by the Food Safety Authority of Ireland (FSAI).[14] They are not medicines. Only health claims authorised under EC Regulation 1924/2006 may be made for them, and any product that promises to "fix" digestion or "treat" a condition is making a claim that is not permitted.
There is exactly one authorised claim in this category. Commission Regulation (EU) 432/2012 permits the wording "lactase enzyme improves lactose digestion in individuals who have difficulty digesting lactose", and only for food supplements providing at least 4,500 FCC units of lactase with each lactose-containing meal, with a note that lactose tolerance varies.[13] No claim is authorised for amylase, protease, lipase, bromelain, papain, alpha-galactosidase, betaine HCl or ox bile. None of the products on this page is marketed under the lactase claim.
Prescription pancreatic enzyme products for diagnosed conditions are licensed medicines regulated by the Health Products Regulatory Authority (HPRA). They are a separate category, available on prescription only, and supplements are not a substitute for them.
VAT on food supplements in Ireland is 13.5%. Revenue's Tax and Duty Manual on food supplement products states that the reduced rate applies to all food supplement products for human oral consumption from 1 January 2020 (paragraph 3A, Schedule 3, VAT Consolidation Act 2010, inserted by the Finance Act 2019), and its list of qualifying substances names enzymes and probiotics explicitly.[16] The Minister for Finance restated the position in a Dรกil written answer on 27 November 2025.[17] Prices on this page include it. - Digestive enzymes are proteins made by the salivary glands, stomach, pancreas and small intestine that break starch, protein and fat into absorbable units.
- Digestive enzymes are not probiotics; enzymes are proteins that act on a meal, probiotics are live microorganisms that act on the microbiome.

- In healthy adults, pancreatic enzymes produced only a modest bloating reduction (p=0.049) after a high-fat meal in one 18-person crossover trial (Suarez et al., Dig Dis Sci, 1999, PMID 10489912).
- Alpha-galactosidase reduced bean-related gas in healthy volunteers (Di Stefano 2007, PMID 17151807) and children (Di Nardo 2013, PMID 24063420) but was no better than placebo in adults with irritable bowel syndrome (Bรถhn 2021, PMID 33619835).
- Within prescription pancreatic enzyme preparations, no product or delivery system was shown superior (Taylor et al., Aliment Pharmacol Ther, 2010, PMID 19804466); over-the-counter enzyme brands have never been compared head-to-head.
- Betaine HCl lowered gastric pH from 5.2 to 0.6 in six volunteers (Yago et al., Mol Pharm, 2013, PMID 23980906); clinical benefit for digestion is unproven and it is unsuitable for reflux or gastritis.

What are digestive enzymes?
Digestive enzymes are proteins that break large food molecules into smaller units the gut can absorb. Amylase acts on starch, protease on protein, lipase on fat, lactase on lactose, and alpha-galactosidase on the oligosaccharides in beans and pulses. The body produces them in the salivary glands, stomach, pancreas and the lining of the small intestine. In Ireland, enzyme supplements are food supplements regulated by the Food Safety Authority of Ireland (FSAI), not medicines.
What enzyme digests starch, protein and fat?
Starch is digested by amylase, first salivary amylase in the mouth and then pancreatic amylase in the small intestine. Protein is digested by proteases: pepsin in the stomach, then trypsin and chymotrypsin from the pancreas. Fat is digested by pancreatic lipase, with bile from the liver emulsifying fat so lipase can act on it. Supplement blends typically supply amylase, protease and lipase together, most often as porcine pancreatin or as fungal or plant enzymes such as bromelain and papain.
Do digestive enzyme supplements actually work?
It depends on who is taking them and why. In diagnosed exocrine pancreatic insufficiency, prescription pancreatic enzyme replacement is well supported: a 2017 Gut meta-analysis of 17 randomised trials (de la Iglesia-Garcรญa et al., PMID 27941156) found fat absorption rose to 83.2 versus 67.4 on placebo (p=0.0001). In healthy adults, the evidence is thinner: Suarez et al. (Dig Dis Sci, 1999, PMID 10489912) found a modest reduction in bloating (p=0.049) after a high-fat meal in 18 volunteers, and two small trials in functional dyspepsia (Majeed 2018, PMID 30156436; Ullah 2023) reported symptom improvement versus placebo. Alpha-galactosidase reduced bean-related gas in healthy volunteers (Di Stefano 2007, PMID 17151807) and children (Di Nardo 2013, PMID 24063420) but not in adults with irritable bowel syndrome (Bรถhn 2021, PMID 33619835). Within prescription pancreatic enzyme preparations, no product or delivery system was shown superior (Taylor 2010, PMID 19804466), and no trial has compared over-the-counter enzyme brands against each other.
When is the best time to take digestive enzymes?
With the meal, usually at the start of eating, because enzymes act on food and need to be in the stomach and small intestine at the same time as the meal. Taking them hours before or after a meal gives the enzyme nothing to act on. Most product labels, including NOW Super Enzymes, direct one capsule with a meal. Targeted enzymes such as alpha-galactosidase or lactase are taken with the specific trigger food. Always follow the label and do not exceed the stated daily dose.
How long can you take digestive enzymes?
Published trials of over-the-counter enzyme blends have run from a single meal (Suarez 1999, PMID 10489912) to two weeks (Di Nardo 2013, PMID 24063420) and 60 days (Majeed 2018, PMID 30156436) without product-related adverse events. There is no long-term safety trial of continuous daily use in healthy adults, so the label directions are the limit that applies. If you find you need enzymes at every meal to feel comfortable, that is a reason to see a GP rather than to keep buying supplements, because persistent symptoms deserve a proper assessment.
Can digestive enzymes make you feel sick or nauseous?
Occasionally. Digestive enzymes are generally well tolerated, but some people report nausea, stomach upset or loose stools, particularly with blends containing ox bile if the meal is low in fat, or with betaine HCl if taken on an empty stomach. Bromelain and papain are derived from pineapple and papaya and are unsuitable for anyone allergic to those fruits, and the NOW label advises consulting a doctor if taking blood thinners. NOW Super Enzymes contains sulphites. Stop taking a product if you react to it, and consult a doctor before use if you are pregnant, breastfeeding, taking medication or have a medical condition.
Do digestive enzymes help acid reflux?
There is no good clinical trial evidence that digestive enzyme supplements reduce acid reflux, and blends containing betaine HCl add acid to the stomach. Yago et al. (Mol Pharm, 2013, PMID 23980906) showed 1,500 mg of betaine HCl lowered gastric pH from 5.2 to 0.6 within minutes in six volunteers. A product that acidifies the stomach is not a logical choice for reflux symptoms. Persistent heartburn or reflux should be assessed by a GP rather than self-treated with a supplement.
Can I take betaine HCl with digestive enzymes?
Many broad-spectrum blends, including NOW Super Enzymes, already combine betaine HCl with pancreatic enzymes, ox bile, bromelain and papain in one capsule, so there is no need to take them separately. Betaine HCl temporarily acidifies the stomach (Yago 2013, PMID 23980906), which is the environment pepsin needs, while pancreatic enzymes work later in the small intestine. A 2020 review (Guilliams and Drake, PMID 32549862) concluded the evidence for meal-time betaine HCl in people with low stomach acid is limited. Anyone with a history of ulcers, gastritis or reflux, or taking acid-suppressing or anti-inflammatory medication, should ask a doctor before using betaine HCl.
Are digestive enzymes and probiotics the same thing, and can I take them together?
No, they are different categories. Digestive enzymes are proteins that act on food during a meal and are used up in the process. Probiotics are live microorganisms taken to influence the gut microbiome over days and weeks. Prebiotics are fibres that human enzymes cannot digest and that feed gut bacteria. They can be taken together and are commonly combined, but a combined low-dose product usually delivers less of each than dedicated products. Take the enzyme with the meal and the probiotic as a separate daily course if both are wanted.
Are digestive enzymes legal in Ireland?
Yes. Digestive enzyme supplements are sold legally in Ireland as food supplements under EU food law, overseen by the Food Safety Authority of Ireland (FSAI).
